Last man rule

Striker has the ball and is running towards the goal.

Goalie has to come out of the zone. Striker does a move, the ball passes the goaly. Goaly extend leg and hits striker on the knee. Striker can't continue running and falls. Ball goes out.

Goalie is the last player. There's no other player from either team close.

Sounds like an easy Red card for DOGSO as long as it meets the 4Ds. What did the referee call in your game?
 
He went to "consult" with the AR... then he called yellow card.

What are the 4Ds?
1) Number of Defenders between attacker and goal
2) Distance from goal
3) Direction attacker is heading
4) Attackers Distance from the ball

Where was the free kick taken from, outside of Penalty Area or PK? If it was a PK, then I could agree with a Caution.

3) Toward the goal
4) Attacker was running with the ball, he punted forward to pass the keeper, at that moment keeper leg's trips attacker

Question on the distance (2), same play, by the midfield (let's say for some reason the whole team was attacking and then there's a counter attack), is it different? In other words, what's the maximum distance to "check" the D for "Distance"?

Well if it was as you describe then it prob should have been a red card DOGSO. To answer your other question....usually the ball at midfield would not meet the D for distance. Those 50-60 yards from midfield to the goal are a long way to dribble the ball without anyone catching you or making a mistake.
